If you have  been seen by another Cardiologist or Specialist your previous records are very important to you care.  Having your records before the visit can make your first visit faster and more complete.  

Please download the records release form and you may email a signed copy to our office ahead of your visit.  We can send it to your other Provider’s office before your visit.  Please call our office to verify the email address to send your signed form to.
